Jake Freeman PMHNP

Psychiatric Nurse Practitioner

I am here for my clients. I believe respect is the cornerstone of patient care and my goal is to partner with clients in the decision making process. I believe in forward progress with compassion.

With psychiatric evidenced-based medication management, my goal is to help you achieve relief from the symptoms that are not working for you any longer. I graduated from University of St. Francis in 2016 with honors attaining my PMHNP license and have worked in the medical field for over 20 years. This experience means I have a deep understand of all areas of health-not just mental health. This is essential when understanding how medications work with your body and brain.

Improving mental health is life long. I currently see clients with major depressive disorder, generalized anxiety disorder, bipolar II disorder, ADHH, OCD, autism spectrum disorder, insomnia, developmental delay and PTSD. My care is inclusive to all patient populations regardless of gender, sexuality, ethnicity from age 12 and older.

My speciality is medication management alongside therapeutic techniques/CBT/BST to help you achieve your goals.
