Treatment Terms, Office Policies & Privacy

Welcome to Clarity Health. This page explains our office policies, your rights and responsibilities, consent to treatment, telehealth, and how we protect your privacy. Please read it fully and do not proceed with care if you do not consent.

Office Policies Patient Rights You have the right to: • Be treated with dignity and respect. • Receive safe, high-quality, individualized care. • Help create and update your treatment plan. • Be informed about treatment options, including complementary and alternative care. • Be informed of risks and benefits of treatment. • Decline or stop a treatment. • Ask questions and have enough time to discuss concerns. Patient Responsibilities You agree to: • Treat staff and other patients with respect. • Take an active role in your care. • Arrive on time, not under alcohol or non-prescribed substances. • Follow the agreed treatment plan or discuss changes with your provider. Clarity Health may end the treatment relationship if: • Our services are no longer helpful or another provider is more appropriate. • You do not pay for services. • You miss two visits without 48-hour notice or miss or cancel frequently. • You repeatedly decline or do not follow the agreed treatment plan. • You or your family are hostile, aggressive, or disruptive. • You no-show and do not contact us within 14 days to reschedule.

Emergencies & After-Hours Care/Emergencies. If you have a psychiatric or medical emergency (for example: suicidal thoughts with a plan, chest pain, sudden confusion), call 911 or go to the nearest emergency department. Additional crisis resources: • National Suicide Prevention Hotline: 1-800-273-8255 (24/7) • Crisis Text Line: text TALK to 741741 After-Hours • We do not provide after-hours on-call coverage. • For urgent but not emergency needs, go to urgent care. • You may message your provider through the patient portal; we aim to respond within 2 business days. • Urgent clinical calls (e.g., serious medication side effects, sudden worsening of symptoms) will be returned as soon as possible by your provider or a covering provider. • Repeated after-hours calls for non-urgent issues (such as refills) may result in a fee. • Texting is not allowed for clinical matters.

Service Limitations & Legal Matters

• Our practice does not handle forensic psychiatry, court appearances, or legal evaluations. • We are unable to offer assessments or professional opinions for matters involving criminal defense, disability claims, worker compensation, or custody disputes. • To maintain the safety of our shared workspace, we cannot treat individuals with violent backgrounds or those who pose a risk to others. • Clients agree to keep Clarity Health and its providers, Jennifer Miller and Jake Freeman, out of any legal litigation. Referrals for forensic specialists are available upon request. Prescription Policy: We provide enough medication to last until your next scheduled check-up. • Safety protocols require a valid appointment for refills; missed visits may pause your prescription access. • Please contact your pharmacist for refill requests so they can initiate the fax process. • Allow 3 business days for processing and 5 business days for insurance authorizations. • Controlled substances are strictly regulated and integrated into a comprehensive treatment plan. • Early refills for controlled medications are never granted. • We utilize the PDMP to track controlled substances; non-compliance with the medication plan may result in immediate discharge. Billing & Insurance: • Fees correspond to the professional time and expertise provided. • Phone consultations incur a $75 fee for every 10 minutes spent (not insurance-eligible). • Documentation, letters, or form completion are billed at $75 per 10 minute increment. • Emergency after-hours calls carry a $75 charge. • Care here is only for those not utilizing government plans like Medicaid or Medicare. • Payments must be secured at the time of booking via a card on file. • This card covers missed visits, late fees, and non-payment collections. • Cancellations with less than 48 hours' notice result in a full service charge. We are largely out-of-network and do not accept government insurance. You may request a superbill for your own insurance claims. Use of AI Scribe As part of the services provided by Clarity Health, we use an AI-powered scribe to assist in the accurate transcription of session notes. The AI scribe is a tool that helps us create detailed records of your consultations. While the AI scribe assists in the note-taking process, the notes are reviewed and managed by licensed professionals. Your health-related data is protected with the same level of security and privacy as outlined in this Privacy Policy. Third-Party Links: Our website may contain links to third-party websites. We do not endorse or control the content or privacy practices of these external sites, and we are not responsible for any damages resulting from your decision to use their services.

At Clarity Health, we are dedicated to protecting your health information. We are required by law to maintain the privacy of your protected health information and to provide you with notice of our legal duties and privacy practices with respect to your health information.

Use and Disclosure of Info

We may use or disclose your health information for treatment, payment, and healthcare operations. This includes coordinating your care with other providers, processing insurance claims, and conducting internal quality assessments. We will only share your information with third parties as permitted by law or with your explicit written authorization, which you may revoke at any time.

Safeguarding Your Records

We implement stringent physical, technical, and administrative safeguards to protect your records. Access to your health information is limited to authorized personnel who require it to perform their professional duties. While we take every precaution to ensure the security of your data within our practice, please be aware that communications via unsecured digital channels may carry inherent risks.

Understanding Your Rights

You have the right to inspect and copy your health records, request an amendment to your information, or ask for a restriction on certain uses and disclosures. You may also request a history of disclosures we have made of your health information. If you believe your privacy rights have been violated, you have the right to file a formal complaint with our office or the Department of Health and Human Services.

We reserve the right to change our privacy practices and the terms of this notice at any time. Any changes will apply to all health information we maintain. If we make significant changes, we will post the revised notice in our clinic and on our website. We encourage you to review this notice periodically to stay informed about how we are protecting your personal health information.

Clarity Health, PLLC is committed to providing a website that is accessible to the widest possible audience, regardless of circumstance and ability. We aim to adhere as closely as possible to the Web Content Accessibility Guidelines (WCAG 2.0, Level AA), published by the World Wide Web Consortium (W3C). These guidelines explain how to make Web content more accessible for people with disabilities. Conformance with these guidelines will help make the web more user friendly to everyone.
